Lesson Objectives:
-
Apply knowledge to real-world cardiac cases.
-
Solve complex case-based scenarios in cardiology.
-
Review high-yield clinical pearls and board-style questions.
Case 1: STEMI in a Young Patient
🚑 34-year-old male with no risk factors presents with crushing chest pain. ECG shows ST elevations in V1-V4.
✔️ Diagnosis? Acute Anterior STEMI (LAD occlusion).
✔️ Management? Immediate PCI + DAPT + Heparin.
✔️ Considerations? Cocaine-induced MI? Check for coronary vasospasm.
Case 2: Recurrent Syncope – Is It Cardiac?
🚑 75-year-old with multiple syncopal episodes, no prodrome, normal neuro workup. ECG shows prolonged PR, wide QRS.
✔️ Diagnosis? Likely high-degree AV block (Mobitz II or 3rd-degree).
✔️ Management? Permanent Pacemaker placement.
Case 3: Sudden Cardiac Arrest – VFib on ECG
🚑 50-year-old collapses in gym, bystander CPR started. EMS finds him pulseless, ECG shows ventricular fibrillation.
✔️ Diagnosis? Sudden Cardiac Arrest due to VFib from underlying CAD.
✔️ Immediate Action? Defibrillation + CPR + Epinephrine every 3-5 min.
✔️ Post-resuscitation care? Therapeutic Hypothermia (if comatose).
